Office Policies
Payment is required for all services at the time they are rendered unless you are in a prepaid plan in which we participate (see below). For those patients, applicable co-payments and deductibles will be collected at the time of the visit. Our office accepts cash, personal checks and Visa/MC/Discover. Patients are responsible for any/all charges not paid for by their insurance company. For out-of-network or uninsured patients, our office will collect the cost of medical (non-elective) services performed, up to a pre-arranged amount. We will then bill your insurance company, apply their payment and refund you the difference or bill you for any remaining balance.
If you must cancel or reschedule an appointment, please do so at least 24 hours before the scheduled appointment time. Your courtesy to Dr. Rotunda and his staff, as well as the other patient for whom an appointment could have been used for, is greatly appreciated.
Dr. Rotunda is currently a provider for:
- Medicare
- Talbert Medical Group
- All major Southern California PPO insurance carriers (ie Blue Cross, Blue Shield, HealthNet, Aetna, Cigna, United/Pacificare, etc.)
